How do I manage the regrowth of my big toenail. ?

Years ago I had a procedure done on my big toe to prevent recurring ingrown toenails. They trimmed both sides of my toenail and applied a solution with a q-tip deep into the sides and it has worked great. Problem is that when the nail grows back it only grows in the middle and becomes very think (.25 to .38 inch thick)and unmanageable. I have recently been able to remove the toenail and clean it throughly. My question is how do I get it to grow back a little more normal and maintain the proper thickness?
