35-year-old healthy female.&#13;
Three months ago, bilateral traumatic great toenail avulsions.&#13;
Now, regrowing, but burrowing into distal nail beds.&#13;
&#13;
Hot, very salty water soaks to decrease tissue swelling.&#13;
Using Japonesque-brand nippers to get under leading edge of nails, pull over tissue.&#13;
Clipping ingrowing shards.&#13;
Almost 24/7 coconut oil application (messy!)&#13;
&#13;
I fear that the nails, which are regrowing strongly, will become ingrown. I have never had an ingrown nail. I have googled the problem and read about cotton bits and dental floss. The toenails are too tight for those.&#13;

Angling the tool under the nails and freeing them is painful and time-consuming but tolerable. I fear I am damaging the nail beds. I also fear I am not freeing the nails frequently enough.&#13;

The bone was hitting my tendon.  She took the bone piece out.  It is not healing and doctor says it will not.  If I am active I have to stay off from it for awhile sometimes a day and it can bother me longer.  It swells, turns red, burns and hurts if I'm on it for extended periods.  

My question...if I don't have surgery will it not get any better?  If I have surgery again what are the chances it will get worse?  I'm not getting decisive answers from doctor.  

What is happening...the tendons and ligaments were suppose to grow in between where the bone was taken out and fuse it together.  It is not.  One side is going up the other down. My ankle had been hurting for a couple weeks before and it was hard to go up any stairs or run. I haven't done much exercise other than a two mile run/jog/walk P.E. last Friday (yes, I did it with my ankle in pain). The weird thing is is that when I 'patted' the other (left) side of my right foot, the right side, where I described before, hurt. Also, when I lift my leg into the air, it hurts in the foot, and when I bend my toes down or up. But, it doesn't hurt when I bend my toes with my hand. I tried Ibuprofen, it didn't help much. I slept ok, and I didn't wake up at night from any pain. When I woke up this morning, it still hurt, but not nearly as much as it did yesterday. I can walk, but I limp. I didn't go to school today, and one of the doctors at my pediatricians' office gave me a 'no P.E.' note, but other than that, she just told me what I already knew. Also, I have been cracking my 'knuckles' (are they the same as on fingers?) on my toes for a couple of years now (I haven't been doing it today because it hurts) , if that gives any reason. But other than that, I can't think of anything that touched my foot other than my socks and shoes. My foot is not disoriented or anything, and I don't participate in any athletic activities, other than P.E.

I just received cortisone shot on my right center of my heel because I'm still in pain. Now, I'm feeling a little tingle and the bottom of my right heel still aches/throbbing.  Now ever since the cortisone my left bottom heel aches and burns.  Also, I'm pes planus, forefoot varus/supinatus bilateral. I also took Xrays, it show no signs of acute pathology on right achillies.  What do you suggest and why I'm experiencing? Also, I have never had this before, I'm not diabetic,  and no injuries. Worst decision I ever made,  I have had recurrent pain and infections ever since,  and still have to cut the nail out every month even though he claimed he chemically cauterized it. I have been treatung it myself since the initial surgery was so painful and a can't afford another multi hundred dollar visit to the podiatrist (that was after insurance, currently I have no insurance).  This latest infection has been going on over a week and I can't seem to curb the pain. After salt soaks and ointment didn't work,  I added drawing salve, which is helping I can see it pulling infection out, but now I'm noticing a ton of granulated tissue that is getting more painful.  This whole experience has led me to a hate and fear of podiatrists and I can't afford another visit.  I would love some options that I can try myself. It's getting painful to walk and that's not an option, I'm a CNA. I'm only worried about it because my Cross Country season is stating up in a couple of week and my ankle needs to be ok to do 35 mile weeks. Two weeks later went to a sports medicine ortho and I was in boot for two weeks, then brace with 6 weeks PT and released without a final xray showing healing.

In March this year, I felt a crunch, crunch , crunch in my left foot/ankle while just standing.  It did not hurt, bruise or really swell.  Two weeks later felt pain while running and realized it felt just like when I broke it.  Went to podiatrist and both ankles of L foot had avulsion fractures.  One partially healed and one not.  Also avulsion fracture of 5th met, that was partially healed.  Put in boot and Dr. ordered bone stimulator.  

4 weeks in my foot hurt really bad.  Asked for crutches and put on them for two weeks.  Made appointment two weeks later appointment and night before appointment finally got bone stimulator.  Went to appointment and was told one break healed other two not.  Told to use bone stimulator and released to do whatever I wanted and to let pain be my guide.  No need for boot or to return unless I had problems.  I specifically asked about gym and running and told no restrictions.

Are bone stimulators really all that great at least 6 weeks or more after the break there is no need for follow up?  Why no final xray to see it healed?  Why not boot etc. if it is still not healed?  Or is it just the way the Dr. handles the ... It may not work but we have to go through the process for insurance purposes and since I only have 12 PT sessions a year it is better to save them in case it doesn't work.

One of them grew back to normal but the other has never properly grown back. It has stopped half way and looks like it's built up and thick compared to the healthy one. 

Reading other websites people are saying its a damaged nail bed and my nail will never grow back. I'm only 19 and get really upset thinking about having to wear a bandaid or covering my toe for the rest of my life. 

Is there anyway this can be fixed? I looked at triple toe straighteners but due to the fact that the tip of second toe is affected, the straightener does not seem to help in the right place. I was wondering if gel toe socks might be better as the toe holders are longer? Or are these meant just to provide relief as toes are repositioned temporarily? I am looking for a treatment not pain relief. My toes are of painful but will become so if this continues. The toes are still flexible so I want them straightened before its too late.
Can you advise me what would be best?

Was given an MRI and finding shows: Barpartite Medial Cuneiform with edema and probable tear of medial itertarsal ligament alongside medical cuneiform. 

I think I was misdiagnosed last year Nov 2012 with a fracture of medial cuneiform. I was put in a boot for 8 weeks and then MRI'd and was told it was a "Non-Union" Fracture of Medical Cuneiform, given a bone stimulator and told surgery was only option to fix a non-union FX or I could try orthotics and "live with it". Since it was Christmas - I chose to postpone surgery and live with it.

However, I recently began having problems with the foot again. If I exercise at all or am on the foot for long periods, it will swell up over the medial cuneiform and ache. Sometimes ache so bad I want to cry. Which prompted me to see a DPM thinking I "re-fractured" it. But I was given this report by the Imaging Center. 

I read this is rare (3 reported cases) and that surgery corrected the problem in two and one rugby player had CT guided injection. However, I read one DPM site where and MD said he had seen and treated 4 himself. just never published. 

So my question is what now??? I like walking...It's kinda important[:)]  and I enjoy exercise, it is vital to health - so what do I do? It sounds like my MD is not very aware of what to do? I would like to go in with some knowledge to help discuss treatment options.

This is in the nature of an odd question, so please bear with me.&#13;

I have undergone a series of 4 surgeries over the last 2 years, Perennial Tendon Debridement, moving of my heel bone, and most recently a triple arthrodesis. I have a phenomenal Orthopedic Surgeon whom I have a ton of confidence in.&#13;
&#13;
Here is my dilemma.  I have been an extremely active executive, with an outstanding employer that has provided me a great deal of time for these procedure and recovery. However, this latest surgery has honestly made (in my opinion) walking much more difficult and pain much more extensive. I feel like I have been diligent in following all protocols and therapy. However, I am at a point where the foot problems are starting to have a significant negative impact on my mental health, and is starting to have a negative impact on my employment. I should also note that the pain is pretty darn extensive, and while medication does help some, I do not want to spend my life on narcotics.&#13;
&#13;
Being an engineer, I have done a bit of self evaluation on how to “fix” the situation. I believe I have reached the point that I would prefer to have the foot completely amputated and move forward to live and function with a prosthesis. From all of my research, it appears that the current state of prosthetics offer a very high level of functionality and quality of life. I am not terribly fond of the notion of losing a portion of one of my limbs, but I believe that if it allows me to return a high level of functionality and better quality of life, it seems to be the correct end state to shoot for. I should note that I am most definitely not a person that has my self worth and self value tied up in body image, so I don’t believe that will have a significant level of impact on my long term mental health.&#13;

I would love any feedback and thoughts you all might have. I am not afraid to have this conversation with my surgeon, but I also don’t want to come off as a complete nut job if I do.&#13;

When I got home I did some more damage carrying heavy things up and down a lot of stairs. I went to the doc the next day and he said it was fractured. I wore a boot for two weeks, then questioned that diagnosis. I went back to a different doctor and he said it wasn't fractured and I could go on as usual. I still felt random pains. I was able to run on it and it didn't hurt terribly, so that tempted me to hit the tennis courts. That was a mistake. That sent me back to the doc in December. I was referred to podiatry. Then I was given a cortisone shot as a trouble-shooting move. That didn't work. Two weeks later I was back only to hear the damage is done and my foot healed wrong. The ligaments were pulled away and that's how they healed. No more tennis - that was a heartbreaker. He said I can still run, just take Motrin and ice after. He said I won't hurt it more. Tennis, it will. That makes sense to me. No sprinting and jumping. He offered fusing it, but that would make me really limited and he didn't think I should either. It was already fused some from the initial correcting of my foot. My question is, am I really doomed with permanent pain and limited sports? I'm having a hard time accepting this.

Thank you in advance for answering my question. I had the all American Procedure performed on a stage 2A right flat foot due to moderate PTTD and a congenital flatfoot (that's my diagnosis). I also have internally rotated knees. The surgery included: a debridement of the posterior tibial tendon, an FDL transfer to the navicular bone, (both the FDL and PTT tendons were kept separate and not tenodesed to each other nor one replaced with the other)--so I now have two tendons controlling the midfoot (at the navicular), the Evan's procedure was also performed, and a medial displacement calcaneal osteotomy was performed. In addition the remaining stump of the FDL tendon at the forefoot was tenodesed to the FHL tendon. While the mid foot arch was corrected and the heel no longer in a valgus deformity, due to the rigid nature of my foot, the adverse effect of the surgery (primarily from the Evan's Procedure) was excessive pressure on the forefoot of the 5th metatarsal and some pressure on the 5th phalanx. In other words, after 5 months post-op, I feel excessive pain on the outside of the forefoot when walking on it. And there is decreased pressure on the 1st metatarsal and halux (on the medial side of the forefoot). So, my forefoot is now at a slight angle: the 1st metatarsal is slightly higher than the 5th metatarsal when weight-bearing -- this is slightly visible to the naked eye but felt very much internally, ie there is more pressure on the outside of the forefoot than the inside, and this hurts! It is not getting better and limits the amount of time I can walk. Also, in order to step on the foot across the 1st through the 5th metatarsals evenly, I now have to slightly internally rotate the knee. And the knee is hurting more now after the surgery than it did before. The internal rotation is also possibly due to femoral anteversion. My questions are: could a cotton wedge osteotomy help bring the 1st metatarsal down equal to the pressure of the 5th metatarsal when weight-bearing? If I try an orthotic and it doesn't work are there any other solutions you can think of? Have you ever seen this problem before with the Evan's Procedure not working correctly due to a more rigid foot?

I was wondering if anyone had any experience with this type of injury or knows more about the healing process. If I am not going to be able to go to Ethiopia for my reasurch next semester I needed to know sooner rather than later seeing as that would mean falling a year behind with my school work and courses. I had on dark red nail polish so nothing was visually wrong (maybe minimally swollen, but don't really recall that).  The next morning, my toe was so sore that I wore flip flops for the ride home.  The next day (Monday), I wore closed in shoes with just a little disomfort, but nothing big.  That evening, I was moving a sofa and stubbed the same toe!!!!  OMG!!!! [:-sweat] I almost passed out!!   It bled profusely and the tip was lifted some.  Most of the bleeding was from the sides closest to the tip end of the toe. I couldn't tell how much it was lifted, because I was too scared to look!!!  I cleaned it as much as I could, applied neosporin, and took Tylenol.  To say the least, it was swollen and bruised.  The following morning (Tuesday), it was REALLY SORE and SWOLLEN.  My 6 year old son was walking ahead of me (wearing the hardest pair of boots in the world).  He stopped dead in his tracks and I stubbed the same toe on his boots!!!!!!  I could hear it tear!  There was bleeding.  I cleaned it, applied ointment, took Tylenol, but had to continue with my day (hopping and moaning).  I still had not removed the red polish.  Acetone anywhere near the nail was just not smart.  There was more swelling.  I utilized Internet medicine vs going to see my dr.  Now, a month since the initial incident, and I finally found the nerve to take the polish off and clip the nail back as far as I think is ok.  There is a really small black "dot" near cuticle and dark red/black hardened "stuff" when you look under the curved, lifted part of the nail.  I still haven't had the nerve to try and lift it to see how much it will come up.  It is still attached at the cuticle and part of the sides.  I don't apply the ointment as much but I keep it clean and cover it since I still haven't attempted to wear shoes.  I will be going to NYC in a week and will need to wear shoes.  My question is:  "Based on the information I provided, will my entire nail fall off eventually?"  "Is it ok to wear shoes?  Will wearing shoes just damage it further?"  "Help! During post op my doc saw that the hardware in the Evans was shifting and causing an impingement.  I also lost most of the arch that was created. Within 6 months after that i had a hammertoe corrective surgery to correct the 2nd and 3rd digit.   Now I am about 2 months out of that surgery and he is suggesting a Cotton Osteotomy and going in and adjusting the hardware from the Evans.  Is this normal to have so many surgeries? Or to have a Cotton Osteotomy to correct a previous Osteotomy? I waited for the dermatology appointment about a month, so it was quite painful by the time I got there.  One Tuesday, he removed it, it felt better (though not painless).  On Wednesday AM, the sole of the foot was swollen.  It does not look red or infected, just fluid filled and I bet this was happening in a milder form before the last removal because it hurt upon wakening the most. I have spasm like pains in the first metatarsal like an involuntary sqeezing while resting. The big toes burns and itches a little. &#13;

I still have a painful infection involving the flesh of the toe. Now it seems like the lateral aspect of the toenail seems to develope the problem, too. I have tried everything from ointments to chemicals but nothing seems to get rid of this painful infection. I change the dressing twice a day to keep it nice and clean, soak it in water and irrigate it. I previously had this pain, but it was caused, I believe, by flip flops. I stopped wearing flip flips and the pain always went away after being off of it through the night. However, the pain I have now, is not going away either with Tylenol or rest. It has stayed below my knee and I feel it everywhere in my lower leg but mainly on the left side of my shin. They trimmed both sides of my toenail and applied a solution with a q-tip deep into the sides and it has worked great.  Problem is that when the nail grows back it only grows in the middle and becomes very think (.25 to .38 inch thick)and unmanageable.  I have recently been able to remove the toenail and clean it throughly. I booked into the first place i found in the local directory.  I arrived to find myself in a natural corrective podiatry centre as opposed to the "run of the mill" podiatrist i was expecting.&#13;
After three visits i have been given "the plan" which involves me visiting very regularly (initially 4 times a week) for at least 6 months.  Obviously the costs for such a plan are very high and if i pay up-front, i can save a fair chunk (although the cost is still close to $1500 AUD). In a fit of frustration, layered my old hard orthotics on top of the new soft ones and the problem is resolving. I have had difficulty with ingrown toenails.  I have had the sides of my big toenail removed and that worked for a while but eventually the nail grew back again.  I had the whole nail removed two years ago...it grew back but dark and thick and hard.  I was trying to remove the ingrown portion yesterday and the entire nail came off.  It detached from under the cuticle.  As a high T-3 paraplegic I struggle with dysreflexia so ingrown toenail, once serious, can cause blood pressure to rise, skin to become splotchy &amp; serious, I MEAN SERIOUS headaches!
