Should heel lifts be soft (eg silicone gel) or hard ?

Also for orthotics - is it better they are cushioned / gel type or hard ?

Depends on what is trying to be achieved.
Soft will flatten under body weight and do nothing, but will absorb impact loads which is important if impact loads are part of the problem.
If the heel lift is to compensate for tight calf muscles, then they have to be rigid.
